«This exploratory study was aimed at developing a better understanding of the quality of care for migrant families including the process of accessing care and receiving collaborative YMH [youth mental health] services as well as the efficiency of such services and the degree of satisfaction with them.» (p. 1937)

Échantillon/Matériau :
L’échantillon comporte 15 participants au total, c’est-à-dire des triades de 5 jeunes, 5 parents et 5 cliniciens. The «[i]nclusion criteria for youths were: being 12–17 years old, coming from a migrant family (either the family or parents having immigrated), and having received mental health services in a collaborative setting for at least 6 months.» (p. 1938) «The study took place in a primary-care, community based health and social service center (CSSS) (Montréal, Canada) that serves a multiethnic population and has three local services centers [...].» (p. 1937)

3. Résumé

«Our results based on the thematic analysis by actor category suggest that migrant youth and their parents are particularly concerned with YMH care settings and how comfortable they are with such venues, as well as with the integration of family issues into treatment. Clinicians emphasize the collaborative aspect of care while focusing on its systemic context. They address the importance of interprofessional and institutional support, while underscoring the need to understand family vulnerability and any sociocultural factors influencing care. […] Based on these results, four themes emerged that should be further researched as potential elements of an optimal care setting for collaborative YMH services for migrant families: (1) establishing an equilibrium between communication, collaboration and privacy/confidentiality, (2) special attention to ensuring the continuity of care and the creation of a welcoming environment where trusting relationships can develop, (3) the inclusion of family intervention, and (4) the provision of collaborative decision-making pathways to care, addressing interprofessional and interinstitutional collaboration as well as cultural differences in explanatory models and values.» (p. 1944)

This study documents perceptions of changes in child and youth mental health (CYMH) services following the reform, as well as facilitators and obstacles to collaboration and partnership in CYMH services, and the specific challenges related to collaboration and partnership when servicing multi-ethnic populations.» (p. 91)

La présente étude est basée sur l’implication de «30 participants, ten from each of the three CSSSs [CSSS de la Montagne, CSSS Cavendish et CSSS Bordeaux-Cartierville-St-Laurent]. […] A focus group was organised with managers of the three CSSSs and with child-psychiatry managers at associated hospitals (seven participants) to discuss issues related to partnership between institutions. […] A joint meeting with workers from the three CSSSs (45 participants) allowed the participants to enrich the interpretation of the preliminary research findings.» (p. 93)

«The reform process encountered challenges in building a common culture of care within and between institutions, while collaboration and partnership evolved in a positive direction throughout the study. Study results highlighted the importance of fostering communication at all levels. Collaboration and partnership was facilitated by opportunities for clinical discussions, dialogue on models of care, harmonizing administrative and clinical priorities, and involving key actors and structures. The results revealed difficulties in implementing multidisciplinary work and in negotiating partners’ responsibilities. Quality of partnership and collaboration appeared particularly crucial in providing optimal care to vulnerable families, including migrants.» (p. 91)

L’étude est basée sur la participation de 30 travailleurs (cadres, travailleuses-social, psychologues, psychoéducateurs, etc.) de différents CSSS de l’île de Montréal. Les auteurs mentionnent également que «[a] focus group was organised with managers of the three CSSSs and with child-psychiatry managers at associated hospitals (seven participants) to discuss issues related to partnership between institutions. Throughout the year, the research findings were presented to the CSSS professionals in half-day meetings at each CSSS (about 20 participants each time). A joint meeting with workers from the three CSSSs (45 participants) allowed the participants to enrich the interpretation of the preliminary research findings.» (p. 93)

3. Résumé

«In general, the CSSS professionals identified the possibility of working with a multi-ethnic population and adapting their care as a positive aspect of their community-based work. They felt that all partners and internal collaborators needed to acknowledge this diversity and to reflect on how to adapt their interventions when needed. The transcultural aspect of many of the child psychiatry consultations was emphasized as instrumental in helping mediate perspectives about care between families and different partners. In addition to child psychiatric consultations, consulting with CSSS colleagues was particularly appreciated in clinical situations in which psychopathology and family dysfunction interacted with migration and cultural issues. Informants felt that working with cultural diversity added complexity in situations where families were experiencing a lot of psychosocial adversity. Interpreters were also seen as important partners.» (p. 95-96) «our study emphasizes that the quality of partnership and of collaboration appears especially crucial to provide optimal care to vulnerable families, including migrants.
